Velopharyngeal incompetence in laymanís term is unintelligible speech.
The usual cause is cleft palate. The palatal muscles responsible for a normal speech, aside from being underdeveloped, failed to fuse in the midline and instead are inserted along the edge of the cleft.
Another cause is incomplete or inadequate or failed cleft palate repair.
Correction of velopharyngeal competence needs proper evaluation including video-endoscopy, and planning. Redo of the cleft palate repair, pharyngoplasty, or use pharyngeal flap may be needed. The appropriate procedure should be tailored according to the patientís needs.
